What company are you shipping with??
You can label with the USPS with no problems.
If you're shipping FedEx, your package might not even be accepted.
I work for FedEx and I've been trying to get employees to understand that with you CAN accept fish shipments. Still if you went to my station and tried to ship fish, most employees will tell you that you can't ship with us because FedEx has a no live animal policy. FedEx considers fish ornamental, and as long they are properly packed to avoid any spills, you CAN ship.
I wouldn't put perishable on the box either, you don't want your box placed in the cargo fridge.
As far as the other labels, I wouldn't even bother. I received a box with fish labeled with "THIS SIDE UP" and "FRAGILE" and saw how the box fell from the top of the pile of boxes on the driver's cart to the floor.

(again) I work for FedEx and packages are covered by FedEx Declared Value policy up to $100 included in price or over if you declare it and pay for it.
As a shipper, YOU need to ensure your package is properly packed and if not, then take it to a packing specialist and have them pack it.
On packing/inspection training, FedEx employees are told to pack or inspect items for shipping and then drop them from a ladder. This is the kind of treatment a properly packed item should survive.
Drivers are not liable for the condition a package arrives, it's the person at the counter if they inspect or pack you item or yourself who refuses to have your package inspected or doesn't pack it properly.
So see? As the shipper, you can't expect your package to be treated like a delicate flower. There's no special foam-lined shelf for Fragile items, they go through the same process than pretty much all the packages.

I doubt they'd go in the cargo fridge but I'm using two heat packs (20 hour and 72 hour) so if they did they'd be heated about 20 degrees higher than the fridge temp. I will be packing my bettas with only the best shipment methods possible...they are going USPS Priority.

Who am I? Well I am a Junior Member of the International Betta Congress and a breeder of white HMPKs, red dragon HMPKs, black/red dragon HMPKs, metallic multi HMPKs, blue masked HMPKs, and now multi color HMs. Soon to add black HMs. I will be showing in New Breeder class in fall of 2012.

Yeah, some companies do put perishables in the fridge :(
I've never had problems with the fish i've received, they've all arrived lively and healthy. You can place all the labels you want with USPS, they actually require you label live fish.
I know they won't be demanding you open the package but I'm sure it's better to label, we don't know what their procedures for these packages are.

Im always afraid.. wont heat packs overheat the water?

Ya but I'm packing them to arrive alive even in subzero temps so if they do...no big deal. I just hope their trip is short...I ship my fry food cultures via Priority and I've never had one take more than 2 days to get to it's destination.

They can get over 150 degrees so it's very important to carefully wrap them in newspaper and protect the fish with plenty of padding.

thats why i dont ship.. id probably not wrap it enough newspaper

Or too little...I will be using phone book paper so I'll probably use two sheets. Also it depends on where you put the heat pack...the bottom of the box will need more news paper than if it was taped to the box lid. Remember that heat packs need air so don't overdo the newspaper!

FFFfffffff the people at my PO were pretty rough with my fish, despite the 'fragile' label on it. After I told them what was inside, they acted like it was going to explode at any given moment. =/

I'm not sure how many postal workers actually...read the labels or take them into consideration.

I just want them to read the HOLD FOR PICK UP label...the others are just to get their attention lol.
