White worms in tank

I have been taking care of my betta in a hospital tank and have been neglecting to take care of the big 10 gal tank I have. I looked in the big one and saw these white worms crawling on the surfaces of the glass. They aren't detritus worms. They're thicker, shorter than the worms. Like leeches. What can I do to get rid of them? I want to do a 100% or almost 100% water change but I'm worried that'll disturb the BB I have in the tank. I also have a sand bottom, do I need to rinse that out or replace it?

If they're planaria, feed less & make sure to do a really good vacuum when doing a water change. I wouldn't do a 100% but do a good water change with a good vac every 2-3 days & they should clear up.

Okay. My betta is is a separate tank so there have been no food in the big tank. Time to bust out the towels and buckets. And take a morning off for for a tank.
