Sudden Fin melt and blindness? Need help!

So, Buddha is giving me another scare, but this time, I'm afraid it's more serious than what I pointed out before.

I noticed about three days ago that he doesn't seem to be able to swim torwards the food 100% of the time. He constantly seems to swim towards the general direction, but often misses it and has to back up and try again. The one time there was a food by his plant, and instead of going around the plant, he insisted on going straight through the plant which didn't lead to progress.

Here is one of his eyes, and the other is pretty much the same.

And that is his color on his head, only noticable in the direct light, which explains the reflection.

Then I noticed, just NOW, his fins were starting to shred! They were perfectly fine a few days ago, but now they seem to be shredding. Normally he has a smooth finline, with no real abnormal ends sticking out.

It may not seem like much, but it worries me because normally his fins are perfect!

Can someone help me figure out what to do?
He is due for a water change, in which I was going to put in AQ salt (for the tear on his fin) as well as the conditioner. Should I do something differently?

Never mind, found out that it just might be that he is in a trance at night, which is whenever I notice the blindness.

and also, it's fin rot so I'm taking necessary precautions now
