Need help!!

I've posted a few threads on my baby Landin but I haven't gotten a clear answer to what's wrong with him.

For months now he has been pale, always laying at the bottom or on a silk plant, and he has slightly ragged fin edges.

He does eat regularly so he is not lethargic. When I tap the glass, he does swim but its aways like he is leaning upward and struggling.

He's been this way for awhile and I still can't figure out what's going on! Please help!

Temp : 78° (has heater)
Food: aqueon pellets and bloodworms. Two pellets a day and a blood worm every other day.
Water Changes: 25% every 3 days and 100% weekly. I add water conditioner.

Three times a week AT THE MOST. Usually 1-2 times a week.

That is one problem only feed them ounce a week and buy some internal parasite killing food.
