Exclamation Fins are patchy and eyes are swollen

Housing

Does your tank have an air stone or other type of aeration? air stone
Is your tank heated? yes
What tank mates does your betta fish live with? ghostshrimp, pond snails

Food
What type of food do you feed your betta fish? Betta bites
How often do you feed your betta fish? 2 pellets morning 2 at night

Maintenance

How often do you perform a water change? once a week
What percentage of the water do you change when you perform a water change? 20%
What type of additives do you add to the water when you perform a water change? API water conditioner, Nutrafin Plant Gro

He jumped out of the tank a few days ago, I had him in a shallow bowl for a day before he was strong enough to swim on his own. I noticed his eyes right away but i just figured they were just irritated from being out of water for almost an hour. His fins have been fine until today they are transparent in a lot of places and have holes all over the place. Ive also noticed some changes in the tank Ive noticed the snails have become very active.

He's got some fin rot and maybe an eye infection too.

Are you sure this tank is cycled? I would invest in a good drops test kit and test your ammonia level. If you see ammonia it is not cycled and you will also need a test for nitrite and nitrate and follow the fish in cycle guide here: http://www.bettafish.com/showthread.php?t=47838
