Location: i live in a tree in the woods in canada
first i put some of his water in his cup them i take the plants out the i put him in the net and put him in his cup then when iam done this time i put the net over a cup then i pored the water in the cup (it went thore the net) so he fell in the net (carefully) then i put the net in the tank and let him swim out

So you're not acclimating him at all..

You shouldn't net him. You should use the cup to scoop him up. Then after you have done the water change (should be same temp water, matched with thermometer under the running tap). you should float him inside the cup in the bowl for an hour while you add a few tablespoons of new water to his cup every 10 minutes.

The way you are doing water changes alone could account for most of his troubles.
