they're usually sedated with clove oil before any such procedure is attempted. it's pretty humane when done properly, the problem that arises, though, is that sometimes people will do it for cosmetic reasons (to "shape" a betta's tail and sometimes pass it off as a naturally occurring shape) using clove oil as an anaesthetic is inherently dangerous since just a little too much turns it into a euthanasia drug, but some people take the risk in order to drive up the prices of their fish, or simply to make something that won't occur naturally.
