Please Help!Noobie in need!

So I recently noticed some spots/odd looking marks on my bettas gills that are a grayish brown. They are very concerning to me as I have been keeping an entry of what hes been doing since I notices some small black markings(lacings?) at the edge of his tail fins. I came down the next day to see a small, triangular chunk of his tail gone where it was formerly marked with the largest bit of black. Im getting concerned, I can try to post the pictures of what he looks like in order of when I got him (about a 20ish days ago) to now. He id a crimson red betta so I dont know if Im over-reacting, he seems happy and has made 2 bubble nests (if that means anything) within the short amount of time he been with me. Please help, Im not sure if I need to do anything.

To post a picture:
  • If the picture is already online:
    Go to the reply box, and click the picture icon. (Yellow sky with gray mountains.) Paste the address of your picture into the box.
  • If the picture isn't online, but is on your computer:
    Click "Go Advanced" at the bottom of the reply box.
    Select the paperclip icon (5th from the right on the top row of icons.)
    Upload the photo from your computer. If the file is too large though, it won't upload.

What tank mates does your betta fish live with? A plant (small, healthy java fern)

Food

How often do you feed your betta fish? 2 am, 2 pm

Maintenance
How often do you perform a water change? weekly
What percentage of the water do you change when you perform a water change? 50%
What type of additives do you add to the water when you perform a water change? conditioner, and let the water sit for a while before its added into the tank
