Wth happened!?!?!?!?!

my beautiful red crowntail died yesterday he started smimming in every direction and then started spazzing and then just died. do any of you know what this is, and i only had him for 2 weeks. his tank was cycled. i fed him 2 times a day 2-4 pellets each time. yesterday i woke up to a weird lump thing on his head his scales were pine-coned only on his head between hid eyes. plz help...
