Dorsal fin gone! Tumor? Infection?

Hoping someone might be able to help me with my betta. Old Blue Has a tumor or some sort of infection. I've been battling it since Nov. Here's my original post:

Lately Old Blue has completely lost his dorsal fin and goes through phases of illness with a white tumor like growth. His appetite is fine but it limits his mobility so he just floats on the top of the bowl. When these flairups happen I do an aquirum salt treatment for 7-10 days.

Any idea what this is? Treatment advice?

Have you used anything besides aquarium salt?

Could you fill out the information on this form, so that we have additional details? http://www.bettafish.com/showthread.php?t=49233

It could be lymphocystis, or a fungal growth. But I'd like to know more about his environment and treatment before recommending anything.

What temperature is your tank? 74-78
Does your tank have a filter? no
Does your tank have an air stone or other type of aeration? no
Is your tank heated? yes
What tank mates does your betta fish live with? none

Food
What type of food do you feed your betta fish? Omega One Betta Buffet
How often do you feed your betta fish? 4-5 pellets a day 6 days a week.

Maintenance
How often do you perform a water change? 1x a week
What percentage of the water do you change when you perform a water change? 100%
What type of additives do you add to the water when you perform a water change? BettaSafe Water conditioner

Water Parameters:
Have you tested your water? no
If so, what are the following parameters?
Ammonia:
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? First started with a lump on his dorsal fin. Then he lost his fin and now there is a white like growth there and is working down his back.
How has your betta fish's behavior changed? Less active. Stays on the top of the tank. Struggles to swim because of the growth and lack of dorsal fin.
When did you start noticing the symptoms? Nov 2012
Have you started treating your fish? If so, how? Aquarium salt treatment 1 x a month or so. Seems to help keep the infection at bay.
Does your fish have any history of being ill? no
How old is your fish (approximately)? 4
