Betta swiming weird and tetra died

Hey Guys,

I've had my betta at work for a bit over a year now but as of about 2 weeks ago i noticed my betta has been real slow and not wanting to swim, constantly laying on the bottom and when he swims to the top he seems to sink quite quickly if he does swim hard.
I did have an issue with nitrate going really high continuesly but this i thought had settled down, I did a 95% water change less then a week ago and already the plants and decorations have mold/algue all over them and on the glass.
and i have just noticed this after noon 1 of my Neon tetra's has died, i did expect this tho as the other fish seem to pic on him.

Tank details
1 betta
4 neon tetras (now 3 )
12L
heated
Filter (changed monthly)
Water change about 50% every 2 weeks.
Feed daily except weekend.

Any help please guys

Housing
What size is your tank? 12L
What temperature is your tank? 24-25 Degrees
Does your tank have a filter? yes
Does your tank have an air stone or other type of aeration? no
Is your tank heated? yes
What tank mates does your betta fish live with? 4 Neon Tetras

Food
What type of food do you feed your betta fish? 3-5 Betta pellets and he seems to eat a couple tetra pellets
How often do you feed your betta fish? daily except no food on weekends

Maintenance
How often do you perform a water change? Fortnight
What percentage of the water do you change when you perform a water change? 50%
What type of additives do you add to the water when you perform a water change? Aqua One Water Conditioner and Stabilizer and PH Down.

Have you tested your water? If so, what are the following parameters?

Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? Looks the same
How has your betta fish's behavior changed? Very slow, seems fatigued, constantly laying on floor/hammock
When did you start noticing the symptoms? 2 weeks
Have you started treating your fish? If so, how? no
Does your fish have any history of being ill? no
How old is your fish (approximately)? ive owned for 1 year

Nitrate: between 20 - 40 PP
pH: 7.6
Hardness: Don't have test
Alkalinity: Don't have test
