Is my Betta dying? I've tried everything! Please help...

No one is replying to my other thread, so I thought I'd try again.. Please help, I think he's dying, and I have no idea what to do...

Housing
What size is your tank? 3 gallons
What temperature is your tank? 75-80 degrees
Does your tank have a filter? Yes
Does your tank have an air stone or other type of aeration? No
Is your tank heated? Yes
What tank mates does your betta fish live with? None

Food
What type of food do you feed your betta fish? Aqueon Betta Food
How often do you feed your betta fish? Once a day

Maintenance

How often do you perform a water change? I do small changes every week, and bigger changes every few weeks
What percentage of the water do you change when you perform a water change? 20% in the small changes and 50% in the big ones.
What type of additives do you add to the water when you perform a water change? API stress coat. I used to add aquarium salt but not anymore.

Water Parameters:
Have you tested your water? If so, what are the following parameters?
I have not tested my water. (However, it's never seemed to bother him in the past...)
Ammonia:
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? He might be a tiny bit bloated but I'm not sure. Other then that he looks the same.
How has your betta fish's behavior changed? Seems to be sleeping all the time... Seems to have a lot of trouble staying afloat. Stays at the bottom of the tank. Completely stopped making bubble nests. Still eats normally.
When did you start noticing the symptoms? Several weeks ago.
Have you started treating your fish? I tried doing a partial change of his water. I tried removing the plants. I tried doing a full change of his water. I tried giving him some Betta Revive. I tried going a few days without feeding him. Nothing is helping!! Is it old age?
Does your fish have any history of being ill? He had fin rot about a year ago, but I treated it and it went away
How old is your fish (approximately)? 1.5-2 years

For a 3 gallon tank, you should be doing at least 2 water changes a week.
How much do you feed him at a feeding?
If he's bloated, but doesn't look like he's pineconeing, you may need to go up to a week without feeding him.
Epsom salts are a good thing to use to help reduce bloating. It will also help with SBD (swim bladder disorder), which may be what he has if he's having trouble staying afloat. Use 1/2 tsp per gallon - remember to dissolve it first before putting it in the tank.
I don't think that 2 years is very old, but if he was a pet store fish, he may be getting old. 3-5 years is the average for Betta fish kept in good to perfect conditions.
