Betta on the bottom of the tank

Housing
What size is your tank? 3.5
What temperature is your tank? 80
Does your tank have a filter? yes
Does your tank have an air stone or other type of aeration? no
Is your tank heated? yes
What tank mates does your betta fish live with? none

Food
What type of food do you feed your betta fish? pellets or something
How often do you feed your betta fish? once a day

Maintenance
How often do you perform a water change? i haven't done any water changes for about a month or two... my last fish died because i did a 100% water change on him and it scared me to do it again. But, I treat the water so no algae grows.
What percentage of the water do you change when you perform a water change? ^
What type of additives do you add to the water when you perform a water change?

Symptoms and Treatment
How has your betta fish's appearance changed? Dull, fins are slowly clamping
How has your betta fish's behavior changed? Laying on her side on the bottom of the tank, breathing hard, her gills are protruding out, and her eyes seem glazed
When did you start noticing the symptoms? 2 days ago
Have you started treating your fish? If so, how? When I noticed this behaviour, I did a 20-30% water change with water I let sit for a couple weeks. I also took out the plants and cleaned them.

I feel like a bad owner. I hate seeing my fish in this condition and want her to survive... please help.

You HAVE to do water changes, that's part of having fish. In a 3.5g you should do two 50% and one 100% change. It's likely poor water conditions.

Fish don't usually die from a water change. If the temperature was too different it could shock him. Or if you forgot water conditioner.

But the only way to keep fish healthy in their water is to do water changes.

With my last fish and the 100% water change, I made sure I put the correct amount of water conditioner and let the water warm up before I put him in it.
I honestly think it's the place I'm staying at, because at my old place I had no problem with the water.

Is there any way to treat my betta fish though or is she doomed?

You could try getting some Prime conditioner as it detoxifies ammonia and nitrates.

You could test the water and see what the base parameters are.

I already use conditioner to take away that stuff, and I have already tested the water as I said in my first post saying it was "safe / neutral". .-.
