For buoyancy problems I would do Epsom salt (Not aquarium salt) 2tsp/gal and tannin treatment in a small QT container that can be floated in the heated tank to maintain temp 76-77F and cover the top with plastic veggie wrap to retain heat and humidity for the labyrinth organ

Pre-mix the treatment water in a 1gal jug of dechlorinated water-Add Epsom salt 2tsp/gal and a tannin source...either IAL (1 med-lrg/gallon) or Oak leaf (10-12/gallon)...let this steep for about an hour to start releasing some of the tannins...shake well before use and use this pre-mixed treatment water for the 100% daily water changes for the next 10-14 days while in the QT container (be sure and attach the QT to the side of the tank so it doesn't sink or get knocked around by the filter)

Nutrition-hold food for 2 days then offer small frequent meals of a high quality varied diet....if you have access to mosquito larva these are great to use

Since he sounds like he has a chronic issue...once he recovery you may need to keep him in Epsom salt 1tsp/gal long term....and increase the regular water changes to twice weekly 50%-

Good luck and keep us posted......
