Whiskey lived with his for about a year? There's really nothing to be done for it. Keep his water clean, keep him comfortable. Another member, CodeRed, had a fish with a similar tumor and eventually he was also euthanized. His was on his back.

I dont want to be cruel, but it seems that your fish suffered from the lack of care. 1 water change a week for a 1 gallon is ridiculous....I do that for a 6 gallon. Also, I would personally invest in a heater. I really do hope he gets better though :(

It does look like either a tumor or a swelling. Change his water with Oldfishlady's recommendation and try adding 1 tsp/gal of EPSOM salt (Mix it in some tank water first to dissolve it, then add it to the tank, then add him). If you're going to use the epsom salt you have to keep up on the water changes, but it's safe to use it long term with bettas.

Only dose the epsom salt for the amount of water you've changed as 3tsp/gal is the max that is known safe.

Also, please try to get a picture of him swimming in his tank so that we can see where it sits on his body.
