Housing
What size is your tank? 5 gallon
What temperature is your tank? around 75 degrees
Does your tank have a filter? yes
Does your tank have an air stone or other type of aeration? I'm not sure. We just have a filter that looks a lot like this:http://source-www.petco.com/assets/product_images/0/015905060806B.jpg
Is your tank heated? yes
What tank mates does your betta fish live with? he's all alone
Food
What type of food do you feed your betta fish? originally pellets, switched to flakes
How often do you feed your betta fish? 2x a day, small meals
Maintenance
How often do you perform a water change? weekly
What percentage of the water do you change when you perform a water change? 20% (1 gallon)
What type of additives do you add to the water when you perform a water change? Seacham Prime (Previously a Top Fin conditioner but were informed this is better)

Have you tested your water? If so, what are the following parameters?
The only one I know is Ammonia, it was high before but ok now

Symptoms and Treatment
How has your betta fish's appearance changed? see attached picture. also had frayed fins.
How has your betta fish's behavior changed? he's way more docile and hangs out at bottom of tank, loss of appetite.
When did you start noticing the symptoms? about a week ago
Have you started treating your fish? If so, how? Yes. Bettafix and Mellafix
Does your fish have any history of being ill? no
How old is your fish (approximately)? We just got him from Petsmart like 5 months ago

Questions regarding your sticky:
Did you attempt to cycle your tank?
I don't know what this means. We've taken all the water out and put new water in the tank.
Do you also check for nitrites and nitrates?
We don't have anything that can check for this but will be taking the water to get tested at Petsmart
I think the water changing schedule needs to be reconsidered once we
establish whether you want to cycle your tank or not.
We're switching to 100% water change every day in order to get him ok.
Hopefully the experts will come along and give you more advice on this. Is it possible to raise the temp to at least 78F?
Yes, it's adjustable. Is 78F the perfect one or something else? I can adjust it if need be.
In the meantime I will do some research on this. Hopefully this will also bump your post up, except now it's quiet time in the west.
