Exclamation Fish suddenly dying

I came home from school 10 minutes ago, and my fish is not well. He isn't really responsive and is just lying at the bottom of the tank. It isn't a 'rest' kind of thing, he looks quite unwell.

Housing
What size is your tank? 3.5 gallon
What temperature is your tank? 78
Does your tank have a filter? Yes
Does your tank have an air stone or other type of aeration? No.
Is your tank heated? Yes
What tank mates does your betta fish live with? None

Food
What type of food do you feed your betta fish? Pellets
How often do you feed your betta fish? 2 in morning 2 at night.

Maintenance
How often do you perform a water change? weekly
What percentage of the water do you change when you perform a water change? 100%
What type of additives do you add to the water when you perform a water change? conditioner and ager

Water Parameters:
Have you tested your water? If so, what are the following parameters?
I did yesterday, and they were all good. Don't remember what they were now.

Symptoms and Treatment
How has your betta fish's appearance changed? slightly cloudy eyes.
How has your betta fish's behavior changed? Still, not moving much.
When did you start noticing the symptoms? This afternoon
Have you started treating your fish? If so, how?No.
Does your fish have any history of being ill? Only from fin rot.
How old is your fish (approximately)? No idea, I got him earlier this year.

Do a small change and be sure to add your conditioner to your water. Sometimes it can be forgotten and the fish will be fine, but other times it can cause drastic issues.

If his eyes are clouding, inspect everything in the tank for any chemicals or colors that could be leaching. Check his heater for any leaks or cracks.

If you find anything suspicious, remove either him or the item from the tank and do an immediate 100% change.

Cloudy eye is usually due to a water quality issue, so just be sure to do extra water changes to help him out. If it gets really bad, then you could start with an aquarium salt treatment and 100% daily changes. If his eyes start swelling, use epsom salt instead. 1 tsp/gal is the dosage.
