Housing
What size is your tank? 10gal
What temperature is your tank? 80
Does your tank have a filter? yes
Does your tank have an air stone or other type of aeration? yes
Is your tank heated? yes
What tank mates does your betta fish live with? none

Food
What type of food do you feed your betta fish? Aqueon Betta Pellets
How often do you feed your betta fish? 2x daily, 2-3 pellets per feeding

Maintenance

How often do you perform a water change? partial: 2x/week, complete: 1x/week
What percentage of the water do you change when you perform a water change? partial: 25-50%
What type of additives do you add to the water when you perform a water change? Stresscoat, aquarium salt as needed

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia: 0ppm
Nitrite: 0ppm
Nitrate: 5ppm
pH: 7.0
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? Some missing scales, one on his left side near his head on the top, one by his mouth on the right
How has your betta fish's behavior changed? Not using right pectoral fin as much as the left, somewhat lethargic (for him, anyways)
When did you start noticing the symptoms? 4 hours ago
Have you started treating your fish? If so, how? No, just monitoring for now
Does your fish have any history of being ill? He's a tailbiter
How old is your fish (approximately)? Rescued from PetSmart at the beginning of May, but was quite young and small when I got him, so...3 months??

Question: Is the tank cycled?
Edit: also, is there anything he could be hurting himself on?

Tank is cycled, nothing he could be hurting himself on unless he rammed himself into an ornament or he's seriously jumping (the lid is mesh to allow for maximum oxygen availability).

If your tank is cycled, there should not be any 100% water changes. That could cause a cycle crash, or a mini cycle. I don't think that could cause missing scales.

Good to know, I've had this particular tank up and running (with a fish in it, anyways, was just sitting before) for less than a week, so I haven't actually gotten to the point of doing a 100% change. You'll have to excuse me, I'm new to bigger tanks.

True. It's not the first time he's lost a scale. In his old tank he managed to whack his head on the filter baffle (I had used a plastic bottle baffle, won't do that again) and scraped a few scales off.
