Help! LES MIS IS IN TROUBLE!

A few days ago I was feeding her and noticed something very unusual, a white splotch on her back. At first I thought it was a pigmentation coloring I had overlooked (unlikely, but I believe it) several days passed and the splotch got bigger and bigger and she started behaving differently. I know what ich looks like and successfully ruled out it being that so instead of treating her with a little bit of everything for everything (I know treating a fish when you don't know whats wrong with them is bad) I started doing about 30% water changes and added aquarium salt for a health boost. Things have gotten worse. What time she isn't lethargic she's swimming into things including the glass and the spot keeps getting bigger. I'm at a loss, please help!

PS she's eating fine, which I find odd because this is her only normal behavior.

Is the spot slightly fuzzy-looking? If you fill out the sticky here:

..that will help a lot in figuring out what's wrong.

My immediate -guess- is typical columnaris. It often starts as a white, fuzzy looking patch on the back.. but pics and that sticky list will really help in making sure.

Yes, it does look a bit fuzzy

Housing
What size is your tank? 10 gallon
What temperature is your tank? A consistent 77 degrees Fahrenheit
Does your tank have a filter? Yes
Does your tank have an air stone or other type of aeration? No
Is your tank heated? Yes
What tank mates does your betta fish live with? None

Food
What type of food do you feed your betta fish? Betta pellets by Aqua and occasionally bloodworms
How often do you feed your betta fish? once a day

Maintenance

How often do you perform a water change? Every two weeks
What percentage of the water do you change when you perform a water change? 25%
What type of additives do you add to the water when you perform a water change? currently just Bettasafe

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia: No current testing kit for these
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? White splotch has appeared on her back
How has your betta fish's behavior changed? She's acting lethargic one moment and spastic the next, scraping against things and such
When did you start noticing the symptoms? About three or four days ago
Have you started treating your fish? If so, how? No
Does your fish have any history of being ill? Not since I've owned her
How old is your fish (approximately)? About 2 years old, maybe a little older
