Benito Needs Help!

I just rescued this betta from my sister, she kept him in really bad conditions for quite some time and I'm trying to help him back to normal, but I need some help.


Housing
What size is your tank? 12g
What temperature is your tank? 80-82
Does your tank have a filter? Yes, a Tetra Whisper 10i
Does your tank have an air stone or other type of aeration? No, it does not.
Is your tank heated? Yes.
What tank mates does your betta fish live with? 2 Ember Tetras

Food
What type of food do you feed your betta fish? I feed him Hikari Betta Pellets, Frozen Blood Worms and he occasionally eats some Omega-One flakes when I feed the tetras
How often do you feed your betta fish? Daily, except for on Sunday.

Maintenance
How often do you perform a water change? At least once a week, but as of lately everyday or every other day.
What percentage of the water do you change when you perform a water change? 25%-50%
What type of additives do you add to the water when you perform a water change? I add Prime with each water change.

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia: 0ppm
Nitrite: 0ppm
Nitrate: >5ppm
pH: 6.8
Hardness: Don't have a test, but I've heard that it's in the lower range.
Alkalinity: Don't have a test, and I have no clue.

Symptoms and Treatment
How has your betta fish's appearance changed? He was like it when I received him a little over a week ago.
How has your betta fish's behavior changed? Not much from what I remember, he does like to hang out in the top corners and float there.
When did you start noticing the symptoms? Before I got him, about 2ish weeks ago.
Have you started treating your fish? If so, how? The only thing I've done is change water. I'm a little scared about putting salt in the tank when it is planted.
Does your fish have any history of being ill? I don't know.
How old is your fish (approximately)? I think he is around 3 years old.

Okay, well how is he acting? Is he rotted up? Lethargic? Really thin? How's his poo look?

He is slightly on the lethargic side, but then he goes on a 10 minute swim around the tank and then swims to his corner and hovers there for a while.

What do you mean by rotted up?

He's not really thin, he's been eating a little less than what my other bettas eat, but it's not too much less.

His poo looks fine.

Oh, sorry. His fins are almost completely black and have receded/rotted back quite a bit. His body has black spots towards his back tail. But that's about it, although I'm not sure if that is considered rotting.

I'm going to go to the pet store to get some salt, and I think that I will try giving him a salt bath. I've been trying to read up on how to help and that's what is says to do. Although, I'm willing to give anything a go for Benito.

How long has he been in your care? I would get salt, and add just a little (1 tsp per gallon, or one tablspoon per 5) and just try to do daily water changes, and ekep an eye on him. All he may need is time :)

AquaKai, I think Sena is trying to get a feel for what exactly is wrong with the fish, since I don't think you really said what the problem is...

Oh sorry! I wasn't really sure what he had. I was thinking that it was fin rot or tail rot, but I wasn't too sure and now looking back I guess that I forgot to add my assumptions. Sorry!

He's been in my care for about a week, but I had left for vacation just after and I had my neighbor feed him. I will do the salt treatment, hopefully it helps him. Thank you!

I've dealt with a very rotted boy before!!! I definitely recommend nice warm and clean water, and salt really does help :)
