Unhappy help fishes keep passing away

I have a huge problem I have had two pefectly helthy bettas crowns one i had for a week the tank was clean the hp was good and it just passed away one day when i went to feed it, the other one i just got the tank was clean hp fine then that night i looked and thought it was dead but it was still alive on the bottem of the tank then it would try and swim and fall again i need to know wgat im doing wrong?? as my crowns keep passing away and my patners crowns that he has are pefectly fine and helths i just dont think i any good at looking after fish like i used to be when i was younger.

Do you and your partner live in the same house? If not then it is probably your water source that is causing the problem.

he lives in the same house witch is why im finding it so strange and we have the same water condiciner in the tanks :s

I am assuming that you and your partner have the same level of care?

What size is your tank?
What temperature is your tank?
Does your tank have a filter?
Does your tank have an air stone or other type of aeration?
Is your tank heated?
What tank mates does your betta fish live with?

Food
What type of food do you feed your betta fish?
How often do you feed your betta fish?

Maintenance
How often do you perform a water change?
What percentage of the water do you change when you perform a water change?
What type of additives do you add to the water when you perform a water change?

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia:
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed?
How has your betta fish's behavior changed?
When did you start noticing the symptoms?
Have you started treating your fish? If so, how?
Does your fish have any history of being ill?
How old is your fish (approximately)?
