Need Some Advice

Hi, First time poster here. My Crown tail Betta has become sick. Here is the disease questionaire.

Housing
What size is your tank? 18 Gallon
What temperature is your tank? 80 F
Does your tank have a filter? Yes - Marineland with bio-wheel
Does your tank have an air stone or other type of aeration? No
Is your tank heated? Yes 80 F
What tank mates does your betta fish live with? None

Food
What type of food do you feed your betta fish? Freeze dried blood worms, Betta Bio-Gold, freeze dried brine shrimp.
How often do you feed your betta fish? Once per day

Maintenance

How often do you perform a water change? 1-2 times per month
What percentage of the water do you change when you perform a water change? 75%
What type of additives do you add to the water when you perform a water change? Seacheme Prime

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia: 0
Nitrite: 0
Nitrate: .5 ppm
pH: 7.2
Hardness: don't know
Alkalinity: don't know

Symptoms and Treatment
How has your betta fish's appearance changed? head turned white/greyish.
How has your betta fish's behavior changed? Lethargic
When did you start noticing the symptoms? became lethargic a couple of days ago, head changed colors a couple of weeks ago.
Have you started treating your fish? If so, how? None yet
Does your fish have any history of being ill? No
How old is your fish (approximately)? I've had him for about 5 months.

I first noticed my Betta's head change color a couple of weeks ago. Looked it up on the Internet and chalked it up to marbling. Now I know that wasn't the case as he now has a dark grey cotton like patch on his back and has become lethargic. I've just completed a 80% water change. He still eats, but not much.

What should i do?

I would suggest moving him to a smaller hospital tank for treatment and for now dose him with aquarium salt (1 tsp per gallon, for no more than 10 days). Could you take a picture of him? That's the easiest way for us to tell what's wrong with him.
