Is my fish dying?

My betta used to be happy. I've had him for over five months now. He lived with me at college, then we took him home and he moved from a small tank to a large, 10 gal one. When I went back to college he came, but his water got nasty because the filter broke. So we got him a new tank (the bio wanted the 10 gal) and he's in a 3 gallon one now. The water is clean, it has a heater, and a filter.
He hasn't been eating for a long time. His back is curved drastically and he looks as if he's just wasting away. I'm very worried about him. He also has black scales all around his front, which used to be blue. His fins are also looking mottled. He mostly lies at the bottom of the aquarium, and it looks as if he is panting. What is wrong?

How old is he?
How often do you clean the tank?
What do you feed him (when he's eating)?
How warm is the water? What temp?

How many decorations/plants? (plastic, silk, real?)

I don't know exactly how old he is, but I bought him about five months back.
When he ate, I fed him freeze dried worms.
I try to take out a quarter gallon of water a week.
His water is around 76 F

Not sure, looks like he is losing scales so he might have tuberculosis but not 100%. It causes scale to fall off and bent spines though. Also rapid weight dropping as well.

He's now laying on his side and not moving. He's panting a lot too. Is there anything I can do?

Yeah I was thinking TB. You may want to go get him euthanized he'll be in a lot of pain. : It sucks but it's for the best. Then you'll need to sterilize his tank so any other fish don't get infected and even you. Don't flush him down the toilet.

Also wear gloves, it's rare but people can catch tb from their fish and wash your hands very good, no open cuts in the water.
