Purely appearance in my experience. Some people like that they are more whole, others crush and bag them like tea. Others simply don't care if they crumble.

How good is C plus grade verses A plus grade. With the C plus I get moss and driftwood.

I have C and most of them came intact. I used to just float the leaf in the tank but now I just boiled them and kept the tannins in the bottle and add a few drops each water change. I also use them to cover up the whole in one of my decor that I don't like my betta going into

I think I will just put them in the tank but how often should I change leaves?

When they sink that means that the tannins have been absorbed. I leave mine in for about two weeks they start to get moldy after
