Setting up a hospital tank for fin rot/bitting

I have a 1 gallon (Minibow) tank that I wanna use just for hospital/quarantine tank with a Top Fin 25W heater.

How would I go about setting it up, to putting the Betta in there?
How long should he stay in the tank?
Does it require any decors/subs?
Is a filter required?
Any other required items to treat him?

Ive never used a hospital tank because my betta fish is on his own. But i did some research cfor u and found some pretty interesting points. The best sites i found are below:

Just for reference, i have found that bettafix is really helpful in curing finrot. Its an allround bacterial antibiotic.
