Consistent Torn Tail

Hello, my sister's betta fish seems to always have his caudal and anal fin torn up. She's told me it's been like this for about 3 months. At first we thought it was fin rot, so she was cleaning the tank twice a week (100%) when she noticed the tail, but she changes it once a week regularly. Since it wasn't going away, there was a decoration in the tank with a plastic spiky plant thing and I told her to take it out because I thought maybe the plant was ripping his fins. She did and there has been no change. Inside the tank she has a rock thing, silk plants, gravel, and small river stones. There is 1 other plastic plant in there, so I don't know if it's the plastic ripping up his fins or if it's fin rot. She's told me it'll heal, but then shortly after it rips up again.

What type of food do you feed your betta fish? NLS and Omega One Betta Buffet Pellets
How often do you feed your betta fish? Once a day (6 pellets)

Maintenance
How often do you perform a water change? Once a week or sometimes twice a week (when the tail is really tattered)
What percentage of the water do you change when you perform a water change? 100%
What type of additives do you add to the water when you perform a water change? Prime and Stress Coat

I think I see the black edges of fin rot.

I think that's a shadow from the camera. In person, the edges of his fins are the blue/clearish like if new fin is growing out. It's just all tattered like if it went through a shredder or something.

Check the ornaments and plants pull them out and drag women's panty hose all through them.. any place they snag will snag your betta and tear fins.

I can't really see anything from the photos.. his fins are too folded or covered.

She couldn't get a very clear picture of him. He's a really hyper fish. I've told her to do that, but she hasn't and she doesn't have any panty hose. His dorsal fin is always curled over. It's always been like that ever since she got him. I think it may be the plant decorations, he does like to hang around the plastic one sometimes. But we also don't know if it's fin rot.

You said that he's hyper, so it sounds to me like he's OK, other than the fins -- is this true?

If he's acting OK (and you're just concerned about his fins) -- can you put a mirror in front of him and get a picture of him flaring?

While he's flaring, he'll probably stay in one place (more or less), which will make it easier to take a picture. And he'll have his fins extended so people here can see them better.
