Shaka isn't looking good... most of his tail fin fell off the other day, after a hole developed in it. Initially, I noticed some small patches on his back (see symptoms below) and what I thought was a scale falling off of the side of his head.. Put him in hospital tank with aquarium salt, but then his fin rot started getting severe and columnaris crossed my mind, so I started treatment with Maracyn 2.
I'm not experienced with betta diseases so I'd really like another opinion on what I should do/what this is. Thank you. I have attatched photos. You can visibly see how severe his tail rot is, as well as the white spot on the side of his head (first image). Please help.

He is currently is a 2.5 gallon hospital tank, but normally lives in a 10 gallon divided

How has your betta fish's appearance changed?
Part (most) of his tail fin has fallen off. He has a white spot on the side of his head. He had some lesions along his spine (looked like scales were scraped off) that are barely visible since treatment.

It has not changed. He is still eagerly eating/flaring/swimming

Has had fin rot since I got him 6 months ago, never has responded to aquarium salt. It got severe one other time, so I treated with Maracyn 2 and it healed.
