Symptoms and Treatment
How has your betta fish's appearance changed? He has become paler and his fins are more rugged. He may possibly have fin rot?
How has your betta fish's behavior changed? When I got him he was very active. I had him in a bowl to start out with for about a month, where he then developed clamped fin. During that time he was very sluggish and would often sit at the bottom of the bowl or float near the top. I then transferred him into a tank with a filter and heater in which his fins reopened within a week. He is more active now, yet still sits at the bottom of the tank pretty often.
When did you start noticing the symptoms? When he got clamped fin a month ago, and the fin rot about a week and a half ago when he reopened his fins.
Have you started treating your fish? If so, how? I got him into the new tank and have been doing weekly water changes as well as adding a small amount of aquarium salt each time.
Does your fish have any history of being ill? Just the clamped fin.
How old is your fish (approximately)? 2-3 months

In February when I got him:
A week and a half ago when he reopened his fins, the bottom and top back fin have split and there is a small hole in the bottom back fin:
