Unhappy Persistent Fin rot - gram positive?

Please help, I've had a VT for six months. Currently 2.5 g hospital tank. 77 degrees. Air pump. 100% water chg weekly plus one or two 50% chgs weekly. He has had slow but almost constant fin rot since I got him. I tried salt(no improvement), MarcynII(no improv), Triple sulfa (some slight improvement in 12 days!), Kanacyn + Furan II (worsened), Melafix + Pimafix (no improv.). He had med breaks in between treatments, but his fins continue to fray & fall off. He has almost no dorsal left. All fins are now affected. He eats well & is active. Could this be a gram positive infection, because all the products I've tried thus far are generally gram negative? Does anyone have any experience or advice? I am afraid I will lose him if this rot doesn't stop. :(

Wow, poor guy....
1) Is he currently on any meds? If so, what? And how long was he on it?
2) Can you post a photo?

He's been on a lot of meds, which is harsh on him. But if the rot is progressing and you want to try something else, Maracyn (Erythromycin) or Tetracyline would both target gram positive infections.

Thanx for responding so much. He is off meds for 1 week tomorrow. He was last on 7days Melafix then 6 days of Melafix + Pimafix, the Kanacyn was for 3 or 4 days alone, then I added Furan II for a total of about 10 days (things worsened), the triple sulfa was for 12 days (the black began to disappear but I didn't want to keep medicating so long), marcynII was the first thing I tried since it worked when I first got him. I used a full course, but no improvement at all. Salt was for 10 days at 1 tsp/gal. I know this is a ridiculous amount of med, but he just keeps getting worse!

I can't use Tetracycline because my Ph is 7.6, & it is ineffective at PhDs over 7.5. I bought Marcyn, but was trying to hold off as long as possible. I've never tried methylene blue, don't know if that is strong enough to stop this. I know most infections are gram negative, but I thought the Kanacyn/Furan would have knocked that out. Below is a pic from a couple weeks ago & one from today.
