New To Site - Sick Fish

Hello,
I got home from work today and my beta, Abyss, did not become active when I when to my room as he normal does. I then noticed him just kinda floating there and he did not come up for food. I waited a bit and noticed him not actively swimming, just stagnant. I became quite worried, he seems to be having trouble getting air, a but of open mouth gasping, not exaggerated gaping, but always moving his lips(?)

I saw this answer thing in the sticky posts so Im going to fill it out:

Housing
What size is your tank?
5 gallon, Fluval chi
What temperature is your tank? 28C
Does your tank have a filter? Yes
Does your tank have an air stone or other type of aeration? No, I dont thing so?
Is your tank heated? Yes
What tank mates does your betta fish live with? None

Food
What type of food do you feed your betta fish?
Top Fin Betta Bites and a treat of a Bloodworm (Omega One Freeze Dried) around once a month.
How often do you feed your betta fish? 5 Days a week, 2-3 pellets

Maintenance

How often do you perform a water change?
Not often, but was cleaned 3weeks ago.
What percentage of the water do you change when you perform a water change? 100%
What type of additives do you add to the water when you perform a water change? Prime Freshwater & Saltwater

Water Parameters:
Have you tested your water?
If so, what are the following parameters?
His water has not been tested.

Symptoms and Treatment
How has your betta fish's appearance changed?
Not a whole lot, his gills seem to not be going in all the way, he might be just a little bit bloated.
How has your betta fish's behavior changed? He is lethargic, didnt want to eat, is just floating not swimming, gulps every now and again, had trouble getting to th top of his tank.
When did you start noticing the symptoms? Today, 11pm.
Have you started treating your fish? If so, how? I placed him in a clean cup of water, so he was closer to the air and no current pushing him around. He has eaten 2 pellets in the cup.
Does your fish have any history of being ill? No
How old is your fish (approximately)? 3-4yrs.

Any suggestions on what to to/what it is? Thanks ^^

I would start with an immediate water change. Even with a 5 gallon tank, you should be doing at least 1 water change a week, it doesn't have to be 100% though, it can be 50%, but it does have to be weekly. He could very well have ammonia poisoning.

Thanks for the info, I will get a testing thing tomorrow, in the mean time he is removed from the tank. He is in a fresh cup with new clean, treated water. What should the ammonia be at? I will clean/boil the rocks, and do a full water and filter change tomorrow before putting him back in the 5 gallon.

Ammonia should be at 0.
