Did the actual liquid portion break and spill into the tank?

Mercury is very, very rarely used anymore unless it's function is absolutely critical, so the vast majority of thermometers are no longer made with mercury. Pure mercury is liquid at room temperature, but it could be mixed with the lead I suppose. I doubt it though.

The lead balls and glass shards will have to be removed and a 100% wc will make that easier.

As stated, it's an alcohol thermometer. (It is not a mercury thermometer.)

However, you don't want alcohol in his tank.

I would do a 100% water change. Dump out the water. Rinse off the decorations. Refill the tank with new, clean water.
