lethargic betta

My betta, Boba, has been lethargic for about a week now. I have saved him twice already when he was close to death. This time he just hangs out near the top of the tank- nose up and sometimes slightly bent to the side. When I approach the tank he usually swims over to where I am or moves when I feed him. Otherwise he just lays there. His color has faded as well. He will be a year old next month. He has a heater and filter in his small tank, some plants as well.

Any ideas? Do his gills look enlarged? what about the color around his neck? Is that OK? thanks- trying to figure out how to post a photo...
