Well your real plants are providing air in the water for snails and other fish (your betta breathes from the top). I don't know why your filter is messed up though o___o maybe it just needs to settle?

I don't have any real aqua plants all i had was a "So called" aqua fern which is actually a Mahogany plant. and Some kind of grass i took both those out.
ive decide to put koi in a temporary home with no gravel till i clean out his tank and filter. he seem to have started to have a hard time breathing as well.

With the cleaning you did, 100% and change in filter media you killed the cycle or any good bacteria that was in the tank.
When the filter clogs, make a 50% water only change and swish/rinse the filter media in that old tank water.
Untreated tap water will kill the good bacteria on the filter media.
Start making 50% water only water changes daily to every-other day for 7 days then do a substrate vacuum with the 50% water change, then stay on a 2-3 times a week 50% water change with one vacuum a week with one of the water changes.
Remove any uneaten food at after feeding.
