Dying fish? Help. :[

Hi, I'm new to the forum. I'm here due to my betta fish, Fishy (so original..) to what I would say being sick. He used to be a really vibrant color and swim happily around his tank. Recently I changed him into a newer tank which he seemed thrilled about, for a week he swam around happily and than suddenly he started floating more to the top and not swim anymore. When you tap the glass he swims downwards quickly but than when he tries staying there he floats back to the top? I'd really like some help with this.

Note: I feed him regularly, his tank is at perfect temperature and the water is checked and everything.

Sorry you are having problems, I have a few question-
How big is the tank, filtration, how much and how often are the water changes and when was the last one, is the tank cycled, water temp and any additives used?
Do you have numbers for your water test and can you post them and also the type of test you are using.
