Unhappy Lump under his throat, and floating to top of the tank

Mister Fishy was acting fine until about a half hour I fed him, I noticed a lump under his throat, and he was floating towards the top of the tank, almost like he couldn't breathe in a way.
I would love to know if it's bad, or what to do about it in general, I'm very nervous!
And for the past about 4 months or so, he's had really bad fin rot, or I think it is. We've used the store bought medicine to treat it, but it seems not to be working. Other than that, he's a really happy little guy, and he's always saying hi to us every chance he gets, he's wonderful! I really want him to get better asap! Thanks.

What size is his home? Filtered? Temperature? When are you doing water changes? How much are water are you changing? How much are you feeding? What medicine did you use?

he has two red stripes on his gills, I don't know if this is his colouring or it could be ammonia burn. Test the water and do a large water change.

But I must say I don't even know if it is something to worry about, I just don't know if it is normal.

Ok.. first of all that betta is borderline emaciated. How often do you feed it?

To better help you we need to know more information:
tank size
water change schedule
feeding schedule and what food you use
do you know what your test readings are?
what medication are you using?

The lump could be any number of things; parasites, tumors, infected gills, etc.
