Shif's Tail Woes

Why would Shif have holes blown in his tail? I noticed a pinhole in his tail a couple days ago and it healed over. Now he has a couple of them, and they aren't just pinholes. Some are a little bigger. Why would Shif have pinholes? >.<

Tank Stats:
2.5 gallon medium critter keeper
Hydor Theo 25W Heater set at 80*
A long strand of hornwort
A little cave with fake but safe plants
Gravel

Did a 100% WC just the day before yesterday and he normally gets a 100% WC twice a week. He's even got a little bubble nest going. Would seeing this little yellow betta stress him out? Even with the new tank covered he's still flaring up a storm and all over his tank... I can't imagine what would be wrong with him and the 5 gallon isn't ready for him yet. >< Should I clear out his tank and dose him with AQ salt?

Still looking for solutions/opinions as to why his tail is getting pinholes. On a lighter note, here's a video of Survivor and pictures of Shif for those that want it.

Are you sure the cave doesn't any rough or sharp edges that could put holes in his tail?

Pretty positive, I tested that cave back when I got it in July and it was fine. He's been happy in it. But the good thing is that the holes in his tail are healed the next day. Here's the thread that has a video of him that I made earlier.
