Sigh... Uncurable Illness?

So, I recently put up a post here asking for some advice about fin rot.
I went out and bought tetracycline for my betta's fin rot, and it hasn't seemed to help much.
He also has a bit of a fungus infection going on too... It's just on his top fin though, but I just noticed a small patch of it on his anal fin...
I purchased some Maracyn 1 today to help combat the fungus. Should I have bought Maracyn 2? And why isn't the tetracycline working? :[
He used to be a lively little fellow, but he seems to be getting more and more lethargic.

Could you please fill this sheet (again)?
It may also be helpful to provide a link to any old threads about the issue so we can get a feel for the whole picture again and perhaps give you some more advice.

Can you describe how the fin rot and the fungus look in detail or perhaps provide us with a clear picture?
