And any pics of the worms you have there? If long, red and stringy sifting around the organic matter, it's detritus worms. Vacuum them off. They're harmless though and simply indicate your tank needs more maintenance. No need to resort to chemicals to kill them off.
He is an apple snail.
The worms usually hang out against the glass,lately mostly at the top.. but every now and then I see one squiggling around in the water.
They don't really have the eyes and triangle head area like a planaria, so I am just assuming they're nematode.
