YES. He NEEDS a heater. It will increase the effectiveness of the meds. and keep him from getting even sicker. Quarantine him for 2 weeks at LEAST.

Do you have invertebrates in the tank?

Pimafix is really not a good med. If you haven't opened it yet take it back and get a good medication like Maracyn. Primafix will not treat bacterial infections which is what they most likely have (body slime infection or columnaris). It only treats fungal infections (and I use the term "treats" lightly since it doesn't really work...). If you can get a real medication then treat the entire tank, including the betta, with the new med. Since the betta most likely has the same infection and just hasn't shown it yet it is important to treat him as well.

Pimafix and melafix is not betta safe and I have some ghost shrimp but I'd kill them all before I'd risk my betta. Its not safe because the oils agitate the labyrinth organ and can cause them to suffocate. Ill try finding some kind of bacterial medicine I only have a petsmart in my area (which is a pet chain store)

Petsmart carries all the medications I listed earlier. They are all betta safe. Maracyn I & II or API Fungus Cure (both treat fungal and bacterial infections and are sold at Petsmart) would be your best bet.

You can remove the shrimp temporarily as they probably aren't carrying it. They are very delicate and will die quickly if infected. They can survive without heat temporarily. You can treat them with Aquarium Salt, 1tsp/gal, just in case.
