White spots appearing on head

I've read through the various diseases and treatments and can't seem to find any that resemble what seems to be going on with Chandler the last few days. First it was this white spot right above his right eye now there seems to be more white spots developing on his face. He also has small white areas showing up on his fins - but that could also be just his coloring. He always flares at me and swims and hides when he see's my phone (my camera). I dont think he's shy - just has a "betta-tude"! So its hard to get a good picture of him but I got one where you can see the white spot on his head.
He also built a bubble nest after the white spot first appeared and he isn't acting sick. But I dont want this to get worse. Has anyone else seen this before?

Housing

What tank mates does your betta fish live with? no

Food
What type of food do you feed your betta fish? flakes - he wont' touch anything else, sometimes won't touch much flakes either!
How often do you feed your betta fish? 2 x a day

Maintenance
How often do you perform a water change? 1-2 per wk
What percentage of the water do you change when you perform a water change? 50-100
What type of additives do you add to the water when you perform a water change? water conditioner/stress coat

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? white spots on face, white showing up in tail, tiny white "sheen" on body near tail
How has your betta fish's behavior changed? none- other than sometimes not being interested in food
When did you start noticing the symptoms? 4-5 days ago
Have you started treating your fish? If so, how? not yet
Does your fish have any history of being ill? no
How old is your fish (approximately)? approx 1 yr

Oh - i forgot to mention. I have a couple of live plants in the tank. One is from a betta bulb - the only one I've managed to actually grow - and another I picked up at Petsmart. Chandler is the only betta I have with live plants in his tank.

I'll try to get a better picture tonight. I'm doing water change for him tonight too so I should be able to get a better pic. I've narrowed it down to either Ick or a fungal infection. I think I'm going to start with some AQ salt with his change tonight.

Ok here is a (hopefully) better picture of Chandler that I got after his water change last night. I used stress coat and AQ salt. I also removed the live plants in case they are the problem. I hope those spots start to disappear!!
