Listless for months

I have a betta in a heated, filtered 5 gallon aquarium. About 5 months ago he got popeye and was treated. Since then there are no outward signs of illness but he is completely listless. He will lay on the bottom until he needs air. He will swim to the surface get air and simply float back down. He has no outward signs and he eats fine so I do not know what to do.

I honestly have two very listless fish myself. They show no signs of disease. They are both hearty eaters, and will sometimes flare at their neighbors for a few minutes-- but they seem to prefer to spend their time laying at the bottom. I've tried salt treatments for a few days, tried frozen foods. Kept their water sparkling. No change.

I'm thinking you may need a slightly bigger water change. I know its less for filtered, cycled tanks but I am not sure what the weekly change amount would be. Maybe 50% a week?

I could be he is just a lazy fish. I have a white delta male that just floats mid level and stares at me. He's alert and eats but isn't at all active. I don't see anyting wrong with him...
I put him in a 5 gallon heated tank with a female next to him and he still just floats and stares at me

If anyone knows the cure for an inactive fish please share it with me!

I have a male that lays on the bottom of his tank nearly every hour of the day. He eats normally, but otherwise shows no real signs of activity.

I have tried anti-bacterial medication, de-wormer, live foods, salt treatments, flaring sessions, and Indian Almond Leaf extract. At the moment I've wedged his tank in between one of my hyper-aggressive males and a female, and still nothing.

If your tank is cycled and you're only registering 5ppm of nitrates, your water change percentages should be fine. Going by your readings, it definitely isn't a problem with water quality.

Perhaps the bout of popeye has compromised your male's immune system in some way. Mine has a badly bitten tail that has never shown signs of re-growth and I wonder if there is some issue with the immune system causing it.
