Mushu - pineconed cheek and white mass??

so last night in feeding i did not notice this so i dont know if its come up over night or been there a few days ive been a little preoccupied with possibly moving and im afraid this has gone unnoticed by my partner or myself. Mushu usually is up the top of his bay and i noticed him on the gravel which is unusual for him and found this. its pineconed on his cheek but no where else i QTd him immediatly.
I also added some AQ salt to the container before floating it in a tank to keep warm

OK info:
Lives: in a divided tank divided between 9 males (custom build)
water pars: all normal (didnt note them down but are all normal)
water: heated and filted with ehiem canistor water change 30-50 % a week
Changes: noticed he was on the bottom of his bay, unusual for him. no change in appetite still a pig :)
Feed: 2-4 pellets a day or small amount of micropellets or thawed bs or bws. fasted one or two days a week

symptoms: pineconing on one cheek with a visible white small mass

Could a swollen gill be causing the pineconing?

Has he been taking anymore atmospheric O2 than is usual for him?

Wow, it looks like he has an infection of the skin on his cheek!! Poor baby =( I'm not sure if epsom salt or aquarium salt would be of most help here, but I'm leaning towards epsom. It might help draw some of that infection out, and keep him from swelling up if it goes internal. Start with that, I'd say about 2 tsp per gallon considering how puffy it is, and tannins/stresscoat if you've got it, and see how he does with that.
