What percentage of the water do you change when you perform a water change?
What type of additives do you add to the water when you perform a water change?

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed?
How has your betta fish's behavior changed?
When did you start noticing the symptoms?
Have you started treating your fish? If so, how?
Does your fish have any history of being ill?
How old is your fish (approximately)?

Did he suffer any type of injury? Did he fall or get stuck in anything? Did he show any other signs of illness? Sores, fin disintegration? Is he thin? Can you get a pic through the side of the tank so we can see the curving better? Can he straighten up at all? Can he get to the surface okay?

I have not seen him straighten up at all. This is the first time that he has ever been non-active. I don't know how old he was when I adopted him. But he is from a relieable local pet shop. I'll get closer pics tonight.

And he gets to the surface when he needs to. He will rest at the top, or in his pot. What gets me the most is when he see's me come in the room he doe's a weak little wiggle.
