bored betta tearing his fIns apart

So I Have A Betta Thats About 2 Weeks Old And This Morning I Saw Him Chasing His TaiL He Was Just Curious At First Then This Afternoon I Noticed He tore Two Big Chunks Of His Fins I Got Really Scared And Went To Petsmart To Buy Him A Buddy A Ghost Shrimp But When I Put The Ghost Shrimp In He Chase It Alotthen I Found The Stressed Little Guy Dead On A Leaf I Cant Move His Plants Because Theyre Alive He Also Has A Bridge In There To Hide

Can you please fill out this form and if possible post us a picture of his fin and his house? it helps us to pinpoint any issues we can adress right away instead of just asking everything one at a time.

If he's bored, add in some colorful plants and ornaments for him to explore! Is his tank bare? That could be the problem. :)

No It Has 2 Live Plants And A Hollow Stone Bridge Hes Always BeEn an Active Fish He Jumped Into His Filter 3 Times before

Live plants aren't too colorful xD. Well you're the one who said he's bored, giving him something to explore or do is the best way to fix that, and that's all I was suggesting.

No I Dont Think Its Stress He Always Goes Up To The Glass And Swims Energetically I Only Have Extra Plastic Plants That Could Also Tear His Fins I Play Games With Him Like FolloW The Finger And Well He Hasnt Figured Ou The Swim Through The Hoop Game When I Leave Yhough He Bites His Tail

Is your tank heated?
What tank mates does your betta fish live with?

Food
What type of food do you feed your betta fish?
What BRAND of food do you feed your betta fish?
How often do you feed your betta fish?

Maintenance
How often do you perform a water change?
What percentage of the water do you change when you perform a water change?
What type of additives do you add to the water when you perform a water change?

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia:
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed?
How has your betta fish's behavior changed?
When did you start noticing the symptoms?
Have you started treating your fish? If so, how?
Does your fish have any history of being ill?
How old is your fish (approximately)?

Water Perimiters Are In Line I Checked Today But Forgot
I Only Give Him 2 Pellets A Day One Morning One night
I Change The Water 2 Times A Week 30%
