Cloudy water?

My cousin's five-gallon tank (I think he has the same as mine) has real cloudy water. His Internet's being a turd so he asked me to do some research. He basically just wants me to ask what he can do to cure it, and if he should remove his betta.

Is the tank filtered? Does he clean it once a week with regular siphoning? What chemicals does he put in it? What does he feed the betta? How long has it been running?

Depending upon the answers, we can figure out why it's clouded. For now, I'd suggest just doing a 50% water change and see if that helps.

It's filtered, he claims to siphon it regularly, and he uses API Stress Coat+ and Ammo Lock. He feeds her Hikari pellets, and it's been going about four months.
