Exclamation Please Help! Sick Betta :(

My Betta's name is Charles... I am new to this Betta Fish forum and have been stalking it for a while trying to find out what is wrong with my beta...
Unfortunately I fear it is many things.
I know for a fact he has fin rot... I believe this is from stress of getting to his new home. (Ive had him for about a month)
I've been doing water changes and everything. I even added a bit of salt.
My question to you lovely people is what else is wrong with my betta??
He has 3 white spots (not salt looking) by his nose... a silver chin and belly.. and one reddish looking streak on one of his deteriorating fins... Im going to include a picture when i can get one...
Thank you to anyone who can offer some advice of how to cure him.
(side note.. he is swimming a lot still and eating fine.)

he has a heater and filter in this one as well as his bigger one. this is his "sick tank" which allows for easier cleaning and better control over temp etc

I just saw this.. so I'm going to fill it out in hope I can get a reply from someone. I"m freaking out :(

Housing
What size is your tank? currently in a gallon "healing" tank
What temperature is your tank? constant 76.
Does your tank have a filter? yes
Does your tank have an air stone or other type of aeration? no
Is your tank heated? yes but the small heater i got won't go above 76
What tank mates does your betta fish live with? none

Food
What type of food do you feed your betta fish? floating beta pellets
How often do you feed your betta fish?once a day. about 4 pellets

Maintenance
How often do you perform a water change? used to do it every other day in his bigger tank. its about 3-5 gallons
What percentage of the water do you change when you perform a water change? sometimes 50... sometimes 25%
What type of additives do you add to the water when you perform a water change? API tap water conditioner. some salt sometimes

Water Parameters:
Have you tested your water? If so, what are the following parameters?
I just ordered some testing kits. I should be getting them soon

Ammonia:
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed? His fins are getting shorter (fin rot) he has a silver chin and face now. He has 2 sometimes 3 white dots on his face (that do not look like salt or fuzzy)
How has your betta fish's behavior changed? He seems normal. Stopped making bubbles for the first time since I had him though
When did you start noticing the symptoms?maybe a week ago.
Have you started treating your fish? If so, how? I bought some Ammonia Safe stuff.. but am nervous to use it because I don't even know if thats the problem yet. I also tried melafix for a while then read that it doesnt work... then i bought Quick Cure cuz it said it fixed protozoan tail rot.. then i read somewhere that that doesn't work either!!
Does your fish have any history of being ill? this is his first illness since being with me
How old is your fish (approximately)? my boyfriend got him from a pet store for me about a month ago.. so idk

I am new to this forum also, and have only had bettas for five months, so the experts could help you a lot more, but I was wondering if your fish could have ich? Here is a link to a website that gives symptoms and treatment for ich-www.peteducation.com/article.cfm?c=16+2160&aid=2421. The spots on the face look like ich. Hope this helps, I know it's distressing to have a sick fish, we've been trying to nurse one of ours back to health for a couple of weeks now-thankfully he's on the mend. Good luck, your fish is a beautiful little guy:)

Ich usually presents as salt-grain looking colonies scattered across the fish's body. I don't think that is ich, but I can't identify it... It could be a fungus... where did all the experts go?! :l
