hi there :3 could you fill this out please? ^-^ if he's too knew to answer water changes and such just use your plans instead. ^-^

Housing
What size is your tank?
What temperature is your tank?
Does your tank have a filter?
Does your tank have an air stone or other type of aeration?
Is your tank heated?
What tank mates does your betta fish live with?

Food
What type of food do you feed your betta fish?
How often do you feed your betta fish?

Maintenance

How often do you perform a water change?
What percentage of the water do you change when you perform a water change?
What type of additives do you add to the water when you perform a water change?

Water Parameters:
Have you tested your water? If so, what are the following parameters?

Ammonia:
Nitrite:
Nitrate:
pH:
Hardness:
Alkalinity:

Symptoms and Treatment
How has your betta fish's appearance changed?
How has your betta fish's behavior changed?
When did you start noticing the symptoms?
Have you started treating your fish? If so, how?
Does your fish have any history of being ill?
How old is your fish (approximately)?

can you tell if it's on the body or only the fins? are his eyes puffy like a pop-eyed goldfish's or cloudy?

also how hard is the plastic plant? you might want to remove it as he could cut his fins on it... if you don't want to use live plants he'll be just as happy with a vase or mug in his tank... but make sure it's never been washed in chemicals, rinse it thoroughly in hot water, no soap or anything, and dry it really well with a clean paper towel.... make sure it's not got paint on it that might bleed off, etc..

and be sure to plant it thoroughly in the gravel, i.e. no shifting or titling, so it can't fall on him...
