Have you started treating your fish? If so, how? Removed Mr. Bubbles and placed him in his old 2.5gal bowl for QT.
Does your fish have any history of being ill? Just being a little bloated here and there and sometimes SBD.
How old is your fish (approximately)? Probably around..8months?

Thank you so much! If pictures are needed, let me know.

Just took out 25% of the water, about to take more. Also, he has live plants. Could this be the source of the problem?

No one has answred yet? o_O huh. Okay, live plants can carry odd critters. Did you quarantine the plants before adding? if so, how long?

Is there more of a description of them? like, are they worm like, round, oval..? Might be planaria?

Heh...no..most of the plants were from his bowl that he had for awhile. I just added Amazon Swords Tuesday. They're all from those tube thingies at Petsmart.

there`s also that picture... and the recommended method for the "plant nemotodes" is to completely clean out the tank, hot water and vinegar for everything (except live plants) including gravel. They latch onto plants...which is why I hate tube plants.... I sugggest quarantine all live plants now, and during quarantine remove ANY waste at the bottom so that they WILL die off. Rinse everything extremely well.

K, how do I do the Water/Vinegar solution? You sure it won't hurt Mr. Bubbles? I'd hate for something to happen to him..
