HELP! Ich or velvet!

My betta fish started acting funny about 3 weeks ago,
Just floating in the top corner of his tank, not eating, and swimming occasionally on his side. At first I though it was swim bladder so I fasted him for a few days.
Now his head is a going white from his lips down compared to its usual deep red. Also when he is breathing quite fast and you can see it all white under his gills when he lifts his head up. His body is starting to go bronze and sometimes he just goes crazy and attacks the side of the bowl. His find are fine.
I tried treating him once with a protozoan medication and have just repeated it.
His tank is over half a gallon and he has a silk plant. He also has a small heater and his temperature is usually 24degrees Celsius. His water is clean and conditioned, I've scrubbed his tank out since he's been like this a few times as well! I also feed him betta pelets. Please help I hate seeing him like he's suffering, what should I do?!

Symptoms and Treatment
How has your betta fish's appearance changed? Colour fading, white under gills
How has your betta fish's behavior changed? Floats in the top corner, bursts of energy attacking the side of the bowl.
When did you start noticing the symptoms? 3 weeks ago when he wasn't swimming much
Have you started treating your fish? If so, how? Fasted him for a few days inase of swim bladder, protozoan antibiotic
Does your fish have any history of being ill? No
How old is your fish (approximately)? 6 months

What I would recommend at this point-make some 50% water changes daily for 2 days and double dose the dechlorinator-be sure the replacement water temp is within a few degrees of the tank water temp and see if this perks him up.

Then get on a regular schedule of 3 weekly water changes on half gallon unfiltered without live plants...2-50% and 1-100% a week to maintain water quality.....

If he doesn't perk up in 2 days or symptoms change-he may need a treatment...but I think maybe improved water quality and double dose dechlorinator may help-unless too much damage has been done and the immune response compromised due to long term water quality issues...

Nutrition-I would fast for 24hr-then offer good quality varied diet in small frequent meals-but since you have mosquito larva-I would offer several rinsed 2-3 times a day....
