My beautiful George has been sick for > 1 year. I have tried many different popeye treatments- no success. He lost one eye already.
I have done treatment for velvet- no success. He has been treated with various antibiotics for so long and I am concerned of his organ function and toxicity.
Housing:

Does it have a heater? yes
What temperature is your tank? 80 F
Does your tank have an air stone or other type of aeration? yes
Does your Betta have tank mates? What kind? nothing
Food:
What food brand do you use?
Do you feed flakes or pellets? pellets- he refuses flakes or other feed
Freeze-dried? pellets only
How often do you feed your Betta? How much? as much as he can eat in 10 mins once a day
Maintenance:
Before your Betta became ill how often did you perform a water change?
What percentage of water did you change? 10 percent Q week then 100 Q month
What is the source of your water? tap
Do you vacuum the substrate or just dip out water? dip
What additives do you use other than conditioner? What brand of conditioner? i don't remember
